PERSONAL INFORMATION

  • Your Social Security Number or Tax ID Number

  • Spouse's full name, SSN or Tax ID, and date of birth

  • Identity Protection PIN (if applicable)

  • Direct deposit info (routing and account numbers)

  • Foreign residency and reporting information (if applicable)


DEPENDENT INFORMATION

  • Dates of birth and SSNs or Tax IDs for dependents

  • Childcare provider info, including their Tax ID

  • Income of dependents or other household adults

  • Form 8332 (if a custodial parent releases their claim to a child)
